Please Help ID a Test Gear CRT

Good Day Gentlemen,

To help a friend on the Yahoo Specialty Newsgroup "Electron Tube Chronicles"
he is tryind to ID the CRT in the two photos shown below.

Description:

- It has a flat face and there is no metal ring.
- It came with a James Millen Mu-Metal shield constructed to fit.
- I Suspect it may be from a Millen 90932 modulation monitor but i can't find
a manual for one of these on the internet.
