It appears to be a wired remote control for an HMV (His Masters Voice) British set. In Britian I believe that Nipper is a trademark of EMI, not RCA. The control appears to have a small speaker so you could listen to the sound at your chair, also volume, brightness, and contrast controls.

Could it be for use in hospitals? The local speaker would maybe fit that use.

I don't think so, I have seen localy at least two Australian TV's with similar remotes that are connected to the set through a very thick wire. They were probably the ultra deluxe, ultra expensive models from the early 60's. Quite rare but probably nobody would pay much for them (at least yet):dunno:
