SAMS Folder

Does anyone have a copy of a SAMS Number 452 Folder 2? I just recently got a B & W 1959 remote controlled model C4007R Zenith TV. The TV works somewhat and likely needs to be recapped. The TV has a Sylvania CRT which means the CRT is not original and therefore likley it has strong emission.
