B&K Model 400 CRT Rejuvenator charts

I am looking for a CRT chart for a B&K 400 CRT Tester/Rejuvenator - the chart you look up the tube and it says what adapter and settings to use. I have checked the BK site, and I can't find a chart for a model that old. If someone knows where I can purchase a chart, that would be good - I am not asking for anyone to violate copyright laws...

Ok, I guess no one knows where I can get a chart... How about this: can someone with a chart for a 400 tell me what adapter and settings to use for a 21ATP4 CRT?
